Usually fully custom starting from a blank (or block) theme, so no unused theme functionality lingers. Building further on an existing theme is possible, but I first check whether that would needlessly complicate the codebase.
Yes, I do this regularly. I move the existing content and structure over to custom block types, usually in phases so the site can stay online during the migration.
